Anguish is rising once more in Sanctuary. Patch 4.3 introduces The Taking, a new main quest marked by eerie disappearances, familiar evils stirring, and the first signs of a far greater scheme at work. This update expands the world with the Rocky Waste, refreshes PvP with Siege of Corvus and Challenge of Equals, adds new Legendary Gems, and delivers a slate of limited‑time events, balance updates, and quality‑of‑life improvements. What begins as an investigation will leave lasting scars on Sanctuary.

As with all previous content updates there will be server maintenance starting on March 18th, from 4-6 p.m. PDT for Oceania, East Asia, Southeast Asia, and Europe servers, and on March 19th from 12-2 a.m. PDT for servers in the Americas. After the maintenance has concluded, all items mentioned below will be live—specific dates for any features going live afterward are provided below.

Investigate The Taking

Anguish is once more taking root across Sanctuary.

In The Taking, a wave of unsettling disappearances ripples outward from Sanctuary’s port cities. Entire communities vanish without warning, leaving only fear and unanswered questions in their wake. Elsewhere, the Rogues’ Eastgate Monastery is beset by horrific manifestations, while beyond the walls of Lut Gholein, demonic forces gather beneath the desert sun.

Following the trail of Andariel, the Maiden of Anguish, you will journey to iconic locations drawn from Diablo’s past, confronting echoes of old evils and uncovering signs of a far greater design at work. What begins as an investigation into isolated horrors soon reveals the opening movements of a larger, cosmic scheme—one that will continue to unfold and shape the fate of Sanctuary in the months ahead.

Content Warning: The Nation in Agony story quest contains depictions of self-harm.

Some evils do not return all at once—first they are felt, then remembered, and only later fully revealed.

Explore the Rocky Waste

The Rocky Waste is the first new subzone arriving this year, expanding the arid deserts that surround the port city of Lut Gholein.

These unforgiving badlands are home to vicious wildlife—spur wolves, dune mantids, seething swarms, and malignant vultures—as well as anguish demons that have begun carving out brutal dominions of their own.

Players exploring the Rocky Waste will find new challenges and rewards through Wanted Monsters and Bounties, offering fresh challenges and rewards amid the encroaching danger. Additional subzones will roll out each quarter as we continue to develop and expand the embattled city of Lut Gholein.

Even the desert feels her passing—anguish takes root long before its mistress is seen.

Battleground Update: Siege of Corvus

Both Classic and Convoy Battleground maps have received a mechanical and visual refresh, centered on a demonic invasion of the ruined city of Corvus.

As the battle unfolds, demonic forces will periodically surge across the battlefield. Teams that press the advantage first will draw the attention—and the power—of these invaders.

Demonic Incursions & Corruption Rewards

  • When a team is the first to push the idols to 25%, 50%, or 75% progress, a wave of demons will spawn across the map.
  • Slaying these demons causes them to drop Demonic Essence for the team that lands the killing blow.
  • Each Demonic Essence grants a stacking 1% damage increase, up to a maximum of 100%.
  • Different demon types drop varying amounts of Demonic Essence.
  • If a player carrying Demonic Essence is slain, they drop half of their stored orbs on death.

Spirit of Corvus

  • Upon reaching 50% idol progress, a powerful boss demon will appear at the center of the map.
  • The team that delivers the killing blow may summon a Spirit of Corvus, allowing one player to call upon its aid to push forward or bolster defenses.

With Corvus once again under siege, victory will favor teams that can balance objective pressure, battlefield control, and the dangerous temptations of demonic power.

New Legendary Gems: Harness the Power of Leviathan Tomb!

Leviathan Tomb – 5 Star Legendary Gem

Dealing damage grants you Abyssal Depths for 6 seconds, increasing your damage done and causing your Critical Hits to grant you Compounding Pressure for 1 second, increasing Crushing Depths’ effectiveness. Additionally, you deal increased damage to enemies suffering from harmful effects. Crushing Depths cannot occur more often than once every 20 seconds, and Compounding Pressure cannot occur more often than once every 1 second during Crushing Depths.

Resonance Gems: The Hunger, Stubborn Oracle, Fading Nostrum, Wulfheort, Golden Filament

Tundra Blight – 2 Star Legendary Gem

Taking damage grants you an aura of Piercing Cold for 6 seconds, chilling nearby enemies, reducing their Attack and Movement Speed and reducing their damage done. Cannot occur more often than once every 20 seconds.

Resonance Gems: Surging Sea, Igneous Scorn, Viper’s Bite

Taxman's Pity – 1 Star Legendary Gem

Your Critical Hits inflict a Grievous Wound on enemies for 6 seconds, reducing the healing they receive and causing them to deal reduced damage to you. Cannot occur on the same target more often than once every 20 seconds.

Resonance Gems: Faltergrasp, Misery Elixir

Enter the Challenge of Equals

You asked for it, and now it’s here! A PvP Tournament for all players, old and new! Whether you’re a PvP veteran, or a newbie, the Challenge for Equals is for you!

Challenge of Equals is a new variant of the Bout of Realms PvP Tournament that normalizes player power to emphasize moment‑to‑moment combat and mechanical skill. Teams of 8 will compete against each other in this new iteration of the ultimate tournament to prove themselves the best in Sanctuary!

  • Sign‑ups begin March 19
  • Tournament runs March 23–27

Power Normalization Rules

Challenge of Equals reduces progression‑based advantages while preserving class identity and core build choices.

  • Legendary affixes and set bonuses remain active
  • Legendary Gem affixes are standardized to Rank 10 effects
  • Five‑Star Legendary Gems are normalized to Two‑Star values
  • The following systems are disabled:
    • Runes
    • Normal Gems
    • Charms
    • Resonance
  • The following bonuses do not apply:
    • Deeds of Valor
    • Legacy of the Horadrim
    • Ancestral Tableau

These changes allow all players., regardless of their time in-game, to make their mark on Sanctuary!

Elite Slayer Loadouts

To support players new to PvP or those looking to experiment, Elite Slayer Loadouts are available in Challenge of Equals.

  • Based on real builds from top contributors in Cross‑Server Bout of Realms and Battlegrounds.
  • All participants can select from this curated roster.
  • Provides immediate access to competitive PvP builds across multiple classes.

Limited-Time Event: Versatile Rings

As corruption reshapes Sanctuary, even its treasures begin to change.

Between March 19, 2026 – May 13, 2026 (server time), all 3+2 and 3+3 quality Rings obtained through any means will feature a versatile socket, allowing Gems of any color to be socketed into them. This temporary enhancement offers unprecedented flexibility when customizing builds and experimenting with new gem combinations.

The versatile socket is only available for the duration of the event and applies to newly acquired qualifying Rings. After the conclusion of the event, Rings with a versatile socket will not be removed and can continue to be used.

Anguish adapts as readily as those who wield it—what once bound power now bends to her will.

Limited-Time Event: Horrid Transformations

For a limited time, familiar horrors will return to Sanctuary… twisted into far more dangerous forms.

During Horrid Transformations, March 19 – April 16 (server time), select zone bosses will undergo terrifying enhancements, becoming stronger, more resilient, and armed with new abilities. These changes are temporary and will only be active for the duration of the event—once it ends, all affected encounters will return to normal.

Enhanced Boss Encounters

Horrid Haunted Carriage / Horried Tax Collector (Ashwold Cemetery)

  • Active: March 19 – March 26 and April 2 – April 9
  • New Ability: Summons minions that tether to the boss and heal it over time

Horrid Ancient Nightmare (Mount Zavain)

  • Active: March 26 – April 2 and April 9 – April 16
  • New Ability: Splits into multiple clones that must be defeated simultaneously

While enhanced, these bosses will feature increased Health and additional attack mechanics, and will spawn every 2 hours each day.

Event Rewards

  • Rewards are granted for the first 6 boss kills per week.
  • The first kill each week guarantees enhanced rewards.

Enhanced rewards include:

  • 2+2 Legendary Item
  • Normal Gems
  • Rare Ore

These transformations are fleeting—but the danger they bring is very real.

Some evils are not content to linger in memory. Under the weight of anguish, even old horrors learn new ways to torment the living.

Limited-Time Event: Suffering’s Rebirth

As Anguish tightens its grip on Sanctuary, new trials arise for those willing to face them.

From March 19, 3:00 a.m. – April 8, 3:00 a.m. (server time), players can complete event-specific tasks to earn Champion’s Commendations, a limited-time currency that can be exchanged for powerful rewards.

Complete Event tasks to earn up to 1200 Champion’s Commendations weekly, and earn valuable prizes, including:

  • Gold ×3,000
  • Scrap Materials ×100
  • Enchanted Dust ×10
  • Legendary Equipment
  • Telluric Pearl
  • Legendary Crest (Bound)

Those who endure the weight of suffering will find themselves well rewarded—but only while the rebirth lasts.

Limited-Time Event: Trading in Blood

Power in Sanctuary is never freely given—it is earned in conflict.

Between March 25, 3:00 a.m. – April 15, 3:00 a.m. (server time), players will take part in a series of limited-time activities to earn progress toward a unified event rewards tracker. Each week brings a new focus, challenging players to prove their strength across shifting battlefields and evolving trials.

This event serves as an evolution of the familiar Event Center experience, offering a more cohesive theme, clear progression, and escalating rewards as the conflict intensifies.

Event Progression Activities

Progress toward the Trading in Blood rewards tracker can be earned through the following rotating activities:

  • March 25 – April 1: Survivor’s Bane (Patch 4.3.0)
  • April 1 – April 8: Trial of the Hordes (Patch 4.3.1)
  • April 8 – April 15: Fractured Plane (Patch 4.3.1)

Each phase introduces new opportunities to advance your standing, with the greatest rewards reserved for those willing to remain engaged as the struggle unfolds.

In the end, only those who spill blood—and risk their own—will claim what is owed.

Customize Your Awakened Wings

The winds of Sanctuary carry many legends; now your wings can tell yours.

After unlocking the ability to restyle your Wings by reaching the heights of 1000 Resonance, visit Yaira at Westmarch to claim your first pair of customizable Wings. From here, you’ll now be able to trade Astral Plumes to re-roll your Wings for new customizations and combinations. Each Astral Plume will cost 160 orbs.

Through this new system, you’ll be able to obtain various wing variations that can roll on 3 different variables: Shape, Material, and Color. On your first roll, only the Color variation is available to roll.

After 2 restyles, you’ll unlock VFX variations, and after that, after 9 restyles, you’ll unlock variations on your wing Shape. Each roll will give you a variation on each unlocked aspect of your wings, giving you virtually limitless combinations.

To give players a little more agency over the wings they’re rolling, you’ll be able to lock one of the aspects of your wings and reroll the other two. For example, you can lock your Color, and just re-roll the shape and VFX.

The more rolls you commit, the more variations of color, shape and VFX you’ll unlock. Each variation also adds to an overall Score for your wings, and reaching higher scores allow you to unlock different perks, including the ability to fly in non-PVP content!

Please note: This feature will not be available in the EU at this time. We are working to determine a release date for our players in the EU, and hope to have it to you as soon as possible.

Wizard

The Breebluk – Offhand – Using a Skill or Primary Attack will briefly reveal you. Detection range increased from 3y to 4y.

Monk

Reticent Wind – Offhand – Invisibility reduced to 1.5 seconds. Movement speed bonus increased to 20%.

Tranquil Palm – Offhand – Animation speed increased to lower untargetable window.

Necromancer

Proximal Fear – Pants – Damage is reduced by 40% while transformed, but Movement Speed is increased to 60%.

Wraith Form – Base Skill – Damage is reduced by 40% while transformed, but Movement Speed is increased to 60%.

Draw Deep – Chest – Range to absorb enemies for a shield reduced from 10y to 5y.

Blood Knight

Verging Curse – Chest – Drain Life bonus changed from a flat 10% to 5% per nearby enemy, up to a maximum of 15%.

Barbarian

Thrashabout – Mainhand – Cooldown increased from 6 seconds to 9 seconds. Attack radius reduced.

Ashes of Antiquity Gameplay Updates

The Battle for the Accursed Tower now supports both PVP (Championship Match) and PVE (Purification Match) victory paths.

Championship Match will retain the current 10v10 PVP format, while Purification Match uses the Accursed Towers format, with activity timing adjusted based on the conflict mode.

Introduced Clan Action Cards, adding new strategic depth through beneficial and negative effects during Accursed Towers activities.

  • Each round has a maximum of 7 cards.
  • 1 card is selected by the Clan Leader during the registration phase.
  • Up to 6 cards can be obtained by participating in Accursed Tower activities.

Using Action Cards

During the Contest Phase, the Leader (or a delegated officer) can operate action cards.

There are two types of cards:

  • Beneficial Effects Cards (3 types): Can only be used on your own Clan.
  • Negative Effects Cards (3 types): Can only be used on enemy Clans listed on the leaderboard (Please note that the leaderboard has an approximately 120 second delay.)

Usage Restrictions

  • A clan can receive a maximum of 4 Negative Effect cards per day.
  • A clan can be targeted by Negative Effects cards from at most 2 different enemy clans per round.

Feature Updates

Set Item Socket Update

  • All Set Items, regardless of quality, will now always roll with maximum sockets.

Familiar Onboarding Improvements

  • New players now receive additional Spirit Essence from Nisza during the tutorial, allowing their first Familiar to start with a full skill loadout.
  • The Gold cost for unlocking the first Familiar battle setup slot has been removed, and a new one-time tutorial has been added to explain Familiar battle setup.

Gem Upgrade Safeguards

  • Added an additional confirmation prompt when using auto-craft to upgrade Gems, clearly indicating when a higher-quality Gem is being consumed.

Gem Resonance Accessibility Improvements

  • The level required to unlock Gem Resonance Slots on all Gems has been reduced by 1.
  • Wing Resonance Reward thresholds have been lowered by 500–1000 Resonance, depending on tier, making Legendary Gem progression more accessible.

Dungeon Experience Optimizations

  • Players who die repeatedly can now exit a dungeon directly from the respawn screen.
  • Performance issues and bugs have been addressed across several dungeons.
  • Added a progression portal in Dread Reaver and Silent Monastery if teleport issues occur.
  • Reduced NPC interaction delays in Destruction’s End and Forgotten Tower for smoother flow.

Gear Quality UI Improvements

  • Legendary and Set Items now feature enhanced item pickup toasts, with higher-quality items receiving more prominent visual presentation.

Gem Upgrade Convenience

  • Players can now purchase Normal Gems directly when upgrading Gems through the Jeweler.

Hero’s Journey Expansion

  • Chapters 5 and 6 of Hero’s Journey are now available, unlocking additional progression and rewards upon completing prior chapters.

Progressive Legendary Gem Upgrades

  • 1 and 2-Star Legendary Gems now feature progressive upgrade paths starting from Rank 4 to Rank 5, aligning with 5-Star Gem progression.

Necromance Summon Enhancement

  • When a Necromancer enters a dungeon instance, their existing summoned creatures will accompany them; there is no longer a need to resummon them.

New Unstuck Mechanism

  • If a player becomes stuck in the terrain while in a dungeon instance, an automatic "unstuck" mechanism will be triggered.

Set Item Update: Vithu’s Urges

  • Vithu’s Urges has been updated to more clearly reward self-applied beneficial effects, while improving consistency with Legendary Gems and Beneficial Effect Duration bonuses.

Updated Set Bonuses

  • 2-Piece Bonus: The duration of beneficial effects you apply to yourself are increased by 30%.
  • 4-Piece Bonus: Applying a beneficial effect to yourself increases your Attack Speed by 30% for 3 seconds.
  • 6-Piece Bonus: Granting yourself a beneficial effect conjures an area of apotheosis at your location for 12 seconds, granting 12% Life Drain to you and any party members within the area. This effect cannot occur more than once every 40 seconds.

Additional Notes

  • The 2-Piece and 4-Piece bonuses now fully focus on self-applied beneficial effects, creating clearer synergy across the set.
  • The updated 2-Piece Bonus now allows all Legendary Gems that grant beneficial effects to have their durations extended by Vithu’s Urges, where previously some effects were not affected.
  • The Beneficial Effect Duration cap has been increased from 80% to 100%.
    • This change reflects that Vithu’s Urges’ 2-Piece Bonus is no longer multiplicative with Beneficial Effect Duration stats, allowing for more intuitive scaling and build flexibility.

These changes reinforce Vithu’s Urges as a powerful option for players who consistently empower themselves and their party through beneficial effects.
